Maximizing Data and Partnerships to Enhance Evidence-Based Program Sustainability

Hear from two Administration for Community Living grantees about their partnership experience with a local health care system and a university to evaluate the impact and return on investment of evidence-based programs. 

YMCA of the Suncoast is a 2018 Fall Prevention grantee who is implementing EnhanceFitness. Working with their local health care partner, Baycare Health System, YMCA of the Suncoast is evaluating the referred patient’s clinical outcomes, health care utilization, and related costs to determine the potential cost-benefit for payors and providers. The results of this evaluation will be used to develop a sliding fee scale for participants and to start contract discussions with local Medicare Advantage plans.  

For five years the Community Council of Greater Dallas (CCGD), also known as CC Advance, has engaged a local university, Texas Woman’s University – Dallas Center, in community-based projects. These projects have focused on studying community needs and their associated solutions.  Success of these projects (e.g., 1 +1 = 3) involves the contributions of the community partners, academic faculty and academic students; in clinical nursing and in health informatics, working together.  This presentation will provide the strategic framework for maximizing the results of the projects and sustaining the community-academia partnership. Outcomes analysis and return on investments that have been realized will be described along with the specifics of the key projects over the years.
