Every senior has a story. Whether your center has improved socialization, physical and mental health, increased exercise and community involvement, or they've participated in an evidence-based program that's changed their life for the better—your senior center has made an impact.

Use this template to collect stories from senior center participants on how your center has positively affected their lives. 

This form can be printed out or filled out electronically. Once completed, please send these stories to membership@ncoa.org so the National Institute of Senior Centers can hear them as well!
