Data Collection Tools

Any organization interested in tracking WWE program data can contact the National Chronic Disease Self-Management Education (CDSME) Database to set up an account. The database is equipped to gather WWE workshop data elements contained in the OAAA pre- and post-test surveys below. With the exception of questions related to physical activity and arthritis management, other health and demographic data mirror the Administration for Community Living’s (ACL) data collection tools, which have been approved by the Office of Management and Budget and are required for active ACL grantees.

Entering Walk With Ease Data

NCOA accepts WWE data into the National CDSME Database, regardless of your funding source. Even if you are not funded by the CDC or OAAA to deliver WWE through a mini-grant, you can enter WWE into the National CDSME Database.

When entering data directly into the National CDSME Database:

  • Workshop Type: Select the specific version of WWE from the following options- Walk With Ease (in-person), Walk With Ease (self-directed), or Camine Con Gusto (self-directed).
  • Once you select a version of WWE, the system will direct you to the Alternate 2 Form for WWE, which will display the fields shown in the program surveys.
  • WWE program data will be reported along with your other program data in Power BI and Salesforce.

Sharing Data with OAAA/UNC

As with all other evidence-based program data in the National CDSME Database, data is used to inform health policy, demonstrate program reach to federal partners, and provide evaluation data to learn more about the effective delivery of these programs. Organizations wishing to share their WWE data with OAAA/UNC, should provide authorization in writing to Tia Crayton (tia.crayton@ncoa.org), permitting us to release the data, along with any restrictions. Data can be sorted at implementation site, program, and participant levels.

Instructions for Recipients of OAAA Mini Grants

If you are the recipient of a mini-grant from OAAA, you must follow the directions and training provided by OAAA, and submit data on a monthly basis to OAAA directly using the template below.
